Background:Zithromax is a widely used antibiotic for the treatment of bacterial infections. However, its impact on patients and healthcare professionals is still controversial. This study aimed to explore the effects of Zithromax on patients with mild to moderate hepatic impairment (n=27), moderate to severe hepatic impairment (n=31), and severe liver damage (n=25) after a single dose of Zithromax for a single week.

Methods:This prospective, randomized, cross-over, double-blind, placebo-controlled study, in combination with the usual care protocol, was conducted to evaluate the effects of Zithromax on patients with mild to moderate hepatic impairment (n=24) and severe hepatic impairment (n=20) after a single-dose Zithromax treatment.

Results:In the Zithromax group, there were no differences in demographic, clinical, laboratory parameters, and laboratory results between the Zithromax group and the placebo group (p=0.9). The most common adverse events were diarrhea, nausea, and vomiting in the Zithromax group, while the incidence in the placebo group was significantly higher than that of the Zithromax group. These adverse events were significantly more frequently seen in the Zithromax group (15-42%) than in the placebo group (2-28%), as well as more frequently seen in the Zithromax group (1-11%) than the placebo group (1%) (p=0.03). A total of 15 patients in the Zithromax group and 17 in the placebo group died during the study (p=0.01). The adverse events related to treatment were severe hepatic failure, severe gastrointestinal bleeding, liver dysfunction, and allergic reactions in the Zithromax group. The most commonly reported adverse events in the Zithromax group were diarrhea, nausea, vomiting, and fever.

Conclusion:This study demonstrated that Zithromax treatment effectively reduced the incidence of serious adverse events in patients with mild to moderate hepatic impairment and severe hepatic impairment and the occurrence of adverse events were significantly higher in patients with severe hepatic impairment and severe hepatic dysfunction compared to the Zithromax group. This study also confirmed that Zithromax therapy is a safe and effective treatment for patients with severe to moderate hepatic impairment and severe hepatic dysfunction.

The median plasma concentration of azithromycin in the Zithromax group was 3.1 µg/L in the Zithromax group and 1.6 µg/L in the placebo group, and the median azithromycin concentration in the Zithromax group was 4.7 µg/L in the Zithromax group and 1.6 µg/L in the placebo group (p<0.001). The azithromycin plasma concentration in the Zithromax group was significantly higher than that in the placebo group (p<0.001).

In the patients with mild to moderate hepatic impairment and severe hepatic dysfunction, Zithromax treatment effectively reduced the incidence of serious adverse events, including diarrhea, nausea, and vomiting. This drug was used to prevent the development of gastrointestinal bleeding in patients with hepatic impairment and severe hepatic impairment, and the risk of bleeding was reduced.
